St. David Athletics
We are enthusiastic about athletics here at St. David Unified School District. It is our mission to give our students the opportunity to participate in healthy sporting activities in a safe, fun, and encouraging atmosphere. We will strive to create activities that are student centered and that display integrity, while being quality-focused and providing a balance in the lives of our students.
2024–2025 Sports Physicals - Mobile Clinic
April 3, 2024
An additional date has been added for the Chiricahua Mobile Clinic to perform school physicals. The clinic will come again on Monday, April 15, 2024. This date is for any St. David students in need of a physical for athletics participation. See Mrs. Jacquez's letter below for scheduling information, payment details, and forms required.
March 25, 2024
An additional date has been added for the Chiricahua Mobile Clinic to perform school physicals. The clinic will come again on Thursday, April 11, 2024. We are filling the needs of HS/MS students first and will open appointments to elementary if any are available. See Mrs. Jacquez's letter below for scheduling information, payment details, and forms required.

Parents of Student Athletes:
If your student plans to participate in any athletic activities during the 2024–2025 school year, they will need an updated AIA physical on file with the office. As in the past, Chiricahua Health Clinic will be providing a major service to help parents make this happen. This is required for both middle school and high school students. Keep in mind that fifth-grade students at times are invited to participate in school sports.

When: Tuesday, March 26, 2024 (More dates to come, I promise!)
Where: Chiricahua Mobile Clinic at the St. David School Campus
- First come/first serve - Call the office to schedule (520)720-4781 extension 107 Heather Richardson
- Students under the age of 14 will need a parent present
- Over the age of 14 will need a signed parental consent form (Notarized or witnessed by two CCHC staff)
- Cost is $5 per physical to be collected by Chiricahua staff the day of the appointment
- Forms filled out prior to appointment and collected by Chiricahua staff; hard copies available in the office
- AIA physical forms (8 pages)
- Consent form - if you cannot be physically present (only if student is over 14)
- Chiricahua forms (if your child is not a patient of Chiricahua Health Clinic)
The plan for March 26 is for middle school and high school students only. Subsequent dates will be open to any students needing a physical.

Requirements for Student Athletes
Before a student may participate in a sport, you must obtain a "white card" by completing the following requirements. See Mrs. Richardson in the front office for paperwork, payment, and if you have any questions.
- Parents and healthcare provider need to complete the physical form. This form is good for the 2023–2024 school year.
- Parent and athlete need to complete the sports paperwork packet available in the front office.
- Middle school students must complete the AIA concussion test and print the certificate at the end. High school students are now required to take both the concussion and opioid tests once per high school career. Athletes must do this once during middle school for middle school athletes (middle school AIA concussion test instructions) and once during high school for high school athletes (High School AIA Brainbook Concussion Course & Opioid Education test instructions). See grade level appropriate instructions for completing the concussion test.
- High school fees: $40 per sport. Middle school fees: $35 per sport.
- Bring your completed physical form, concussion certificate, paperwork packet, and payment to our activities secretary in the front office. Make checks payable to St. David School. For electronic payments, please use the Skookii link on the left.
