St. David Athletics

We are enthusiastic about athletics here at St. David Unified School District. It is our mission to give our students the opportunity to participate in healthy sporting activities in a safe, fun, and encouraging atmosphere. We will strive to create activities that are student centered and that display integrity, while being quality-focused and providing a balance in the lives of our students.

June 12, 2021 Sports Physicals

RE: 2021-22 Sports Physicals

If your student plans to participate in any athletic activities through the school for the 2021–2022 school year, they need an updated AIA physical on file with the office (we waived last year due to COVID). As in the past, Chiricahua Health Clinic will be providing a major service to help parents make this happen. If your child will play middle school or high school sports, this is required. Please keep in mind that we occasionally invite 5th graders to participate.

The mobile clinic will be on the St. David School campus on Thursday and Friday, June 24 and June 25, from 8:00 a.m. until 5:00 p.m. with a medical provider to complete the physicals. Please call the school office Monday through Thursday from 8:00 a.m. until noon to schedule an appointment. This will be first come, first serve for the appointment times. If your child is under 14, a parent will need to be there for the appointment. If your child is over 14, you will need to provide a signed consent allowing for the physical to be done without a parent being there. The cost of the physical is $5. You will need to print and fill out the AIA physical forms (total of five pages), and if Chiricahua Health Clinic does not see your child, a registration form, financial form, and privacy form. We will have copies of these forms available in the front office, but they must be picked up and completed prior to the appointment date due to time constraints. We will not be collecting the $5 or paperwork prior to the scheduled appointments but expect that the students will arrive with them in hand and completed. Also, you will not get a reminder phone call so please, make a note of your student's appointment!

Again, this is a huge benefit to our student-athletes and parents! We are grateful to Chiricahua Community Health Clinics for making this happen!

Requirements for Student Athletes

Before a student may participate in a sport, you must complete all of the following items. 

  1. Parents and healthcare provider need to complete the physical form. This form is good for the 2020–2021 school year. 
  2. Parent and athlete need to complete the sports paperwork packet available in the Front Office
  3. Complete the AIA concussion test and print the certificate at the end. You must do this once during middle school for middle school athletes (middle school AIA concussion test instructions) and once during high school for high school athletes (high school AIA concussion test instructions). See grade level appropriate instructions for completing the concussion test. 
  4. High school fees: $40 per sport. Middle school fees: $30 per sport. 
  5. Bring your completed physical form, concussion certificate, paperwork packet, and payment to our activities secretary in the front office. Make checks payable to St. David School. For electronic payments, please use the Skookii link on the left.
