St. David Athletics

We are enthusiastic about athletics here at St. David Unified School District. It is our mission to give our students the opportunity to participate in healthy sporting activities in a safe, fun, and encouraging atmosphere. We will strive to create activities that are student centered and that display integrity, while being quality-focused and providing a balance in the lives of our students.

April 6, 2021 Annual Physicals

Hello wonderful Tiger parents!

I would like to just give a quick up-date on the annual physicals that we have offered in the past on campus for the following academic/athletic school year. The past few years, Chiricahua mobile clinic has come and now having a nurse practitioner has completed any and all physicals needed, both for middle school and high school for only $5.00. I have been in touch with Chiricahua Clinic and due to COVID-19, they are not currently offering the mobile clinic as an option. I will remain in touch with them and if this becomes an option during the summer, I will do my very best to arrange it and let everyone know.

In the meantime, please keep in mind that if your student, middle school or high school, plans to participate in any athletics in the 2021–2022 school year, they will need a new physical. We have waived this requirement this past year, allowing 2019–2020 physicals to be in use due to COVID but this will not be the case again.

Attached you will find the 2021–2022 AIA pre-evaluation physical form and the annual physical evaluation form. If you choose to take your child to their primary care provider, please have both of these forms completed and signed and return to Heather Richardson in the office. Hopefully the mobile clinic will be an option during the summer, but please keep in mind that fall sports typically start before the beginning of school so don’t wait until the last minute. Time flies, especially summertime!

Whether they are on or off campus, our fall athletes have shown a dedication and commitment worthy of praise! 

Requirements for Student Athletes

Before a student may participate in a sport, you must complete all of the following items. 

  1. Parents and healthcare provider need to complete the physical form. This form is good for the 2020–2021 school year. 
  2. Parent and athlete need to complete the sports paperwork packet available in the Front Office
  3. Complete the AIA concussion test and print the certificate at the end. You must do this once during middle school for middle school athletes (middle school AIA concussion test instructions) and once during high school for high school athletes (high school AIA concussion test instructions). See grade level appropriate instructions for completing the concussion test. 
  4. High school fees: $40 per sport. Middle school fees: $30 per sport. 
  5. Bring your completed physical form, concussion certificate, paperwork packet, and payment to our activities secretary in the front office. Make checks payable to St. David School. For electronic payments, please use the Skookii link on the left.
